Students who attend TAG meetings will receive additional volunteer opportunities throughout the year!

Note: There is no charge for the Commissioner of Oaths service for Change of Name forms.

The Municipality of Clarington offers Commissioner of Oaths services. Certain documents require a Commissioner to certify that a required oath, affirmation, or declaration have been administered properly. The Municipality's Commissioner of Oaths will witness the signatures of people who are affirming/swearing the contents of a document to be true for a fee of $28.25 payable by debit or cheque.

Please provide valid photo identification to have your signature witnessed by a Commissioner of Oaths. NOTE:  There is no charge for the Commissioner of Oaths service for change of name forms.

Common documents the Commissioner of Oaths can witness include:

  • Consent letters for children travelling out of the country
  • Vehicle transfers
  • Immunization exemptions
  • Insurance claim forms

Examples of documents the Commissioner of Oaths cannot witness include:

  • Wills
  • Power of attorney documents
  • Divorce papers or separation agreements
  • Real estate documents, including transfer of land

If you are in need of Commissioning services, and you are unable to attend the Municipal Administration Centre during business hours, you can attend one of the extended hours service/pop-ups.

Commissioning of the Oath will also be held on Saturday, November 25, 9:30 a.m. to 12:30 p.m. at the Courtice Library.

Advanced registration required for each 20 minute session through the Tri-Regional Infant Hearing Program at www.childdevelopmentprograms.ca/bookmybaby or 1-888-703-5437 x4.

Children develop speech and language skills by listening and imitating those around them, but these skills may be delayed in the case of deafness or hearing loss.

Sign up your infant for a screening if you have hearing-related concerns, and learn about the follow-up services available to your family.

Presented in partnership with Markham Stouffville Hospital, the lead agency for the Tri-Regional Infant Hearing Program.
