The Durham Community Health Centre's Hepatitis C Outreach Program, funded by the Ministry of Health, is one of 24 hepatitis teams in Ontario, serving Durham Region and four surrounding counties.

The initiative provides free education, testing, and treatment through a test-to-treat model using mobile units. A clinical team, including a hepatologist, registered nurse, case manager, and outreach workers,delivers personalized care from screening to treatment completion, with monthly site visits to remove barriers and improve health outcomes.
